Active Life Adult Day Health Care Center

Active Life ADHC is a licensed and Medi-Cal certified CBAS Program (Community Based Adult Services) center that provides medical and social services in a daytime setting for adults ages 18 and up, including the frail elderly. We work closely with caregivers and family members to help our program participants stay living at home, delay institutionalization, minimize hospital visits and more. This vital program provides valuable respite to families and caregivers who can drop off or have their loved ones picked up for four hours of supervised activities and therapy each day of attendance. Available to eligible beneficiaries who are enrolled in a Medi-Cal managed care plan such as Health Plan of San Joaquin, Health Net, Kaiser / Medi-cal.
